DIBH-App: a digital tool to support Patients with breast cancer to practice deep inspiration breath hold (DIBH) at home before radiation therapy treatment

Deep inspiration breath hold (DIBH), reduces radiation of the heart and lung. Effective implementation requires patient preparation and consistent performance, which can be challenging in clinical settings. A digital tool—the DIBH‑App—was developed to facilitate home‑based practice. It was complemented by a refined breathing protocol, Deep Inspiration Thoracic Expansion (DITE) with deep expiration and maximal chest-wall movement. The study aims to evaluate whether a person-centered DIBH training model, combining the DIBH-App and the DITE protocol, improves patient preparedness, reduces distress, and enhances treatment efficiency compared with standard care. In this trial, patients scheduled for radiotherapy with DIBH will be randomized to either standard care or access to the DIBH‑App. Data collection includes validated questionnaires (distress, health literacy, preparedness), logged app usage data and CT /RT-data. Qualitative interviewes will be performed.

Patients > 18 years of age with a left-sided breast cancer diagnosis or DCIS who are to receive RT with DIBH technology with curative intention; preparatory CT visit minimum 4 days to a maximum of two months able to speak and understand enough Swedish to be able to assimilate content in mobile phone
Patients receiving palliative RT or neoadjuvant chemotherapy; Planning CT starting < 4 days; as well as people with some form of orthopedic, rheumatological or neurological injury or disease that limits the possibility of carrying out the intervention.
