The purpose of this study is to investigate the efficacy of immediate, curative therapy in terms of overall survival and quality of life compared to conservative treatment in older men (≥75 years) with high-risk, non-metastatic PCa. Treatment effects on several secondary and exploratory endpoints will also be investigated.

Male, 75 years of age or older capable to sign informed consent.
Healthy as determined by medical evaluation and geriatric G8/miniCOG™, and who had a high-risk PCa diagnosed <6 months, previous low-risk and intermadiate risk may be allowed.
Dementia, prior radiation to the pelvis, hormone therapy >3 months prior randomisation. Lymph node metastasis (N0). Distant metastasis. Disabled or severe comorbidity. Unable to read, understand or fill out questionnaires.
